I hereby consent (i) to the collection, recording, organization, storage, updating, retrieval, consultation, use, consolidation, transfer, sharing, and/or processing of LESSEE’s personal information and sensitive personal information, including those of LESSEE’s owners, partners, officers, employees, shareholders, contact persons, authorized signatories/representatives, and references (“LESSEE’S Representatives”) provided by LESSEE, by the LESSOR and its related corporations and affiliates (collectively, "SM Prime Group"), their agents and authorized service providers, in order to review, process, implement or perform acts in relation to the tenant application form, tenant profile form, contract of lease, including its annexes, supplements, amendments, and such other instruments, papers, or documents submitted by LESSEE in connection therewith and to process and/or respond to LESSEE’s request, query, feedback or comments and (ii) to the SM Prime Data Privacy Policy found at https://smprime.com/smphi-privacy-policy.
I represent and warrant that I have the power and authority to consent on behalf of the LESSEE to the data privacy policy herein referred to, as well as consent to all terms and conditions mentioned herein and that this consent was given by the LESSEE and is binding upon the LESSEE. LESSEE further represents and warrants that it has obtained the express consent of the LESSEE’s Representatives who personal information was provided by LESSEE to SM Prime Group and that, in the event that LESSEE’s Representatives revokes, withdraws or otherwise modifies such express consent, LESSEE shall inform SM Prime Group in writing.
